    AH-0064 Nurse Practitioner / Advanced Nurse Practitioner

    Location: Access Health Care, Multiple Locations across Plymouth

    Salary: £ -£ pro rata – dependent on experience

    Full or part time (hours negotiable). Full time working to cover over 5 days; however Job share or Part-time working will be considered

    Access Health Care is looking to recruit an enthusiastic Nurse Practitioners / Advanced Nurse Practitioners who have experience working with patients in an acute setting as well as the multidisciplinary healthcare team. This is a new and exciting opportunity to expand our nursing team in a primarily patient-facing role, with the chance to utilise your clinical skills to improve the health of the local community.

    You will be involved in the integrated Practice multidisciplinary teams, specifically working within the Acute Care Team at Stirling Road Surgery and also provide face to face appointments across our other practices in Plymouth. You will also be involved in staff training and development, clinical audit, developing policies and procedures to meet the needs of patients.

    The responsibilities will include, but are not limited to the provision of acute ‘on the day’ care for our patient group:

    • Assessing, diagnosing, planning, implementing and evaluating treatment and care for patients presenting with an undifferentiated diagnosis.
    • Clinically examining and assessing patient needs from a physiological and psychological perspective, and planning clinical care accordingly.
    • Proactively identifying, diagnosing and managing treatment plans for patients at risk of developing a long-term conditions (as appropriate)
    • Providing assessment, screening treatment, services and health education advice.

    We are looking for candidates who are adaptable to change, with excellent clinical skills and a good level of competence in the use of IT. Candidates should be able to demonstrate excellent organisational skills and an ability to communicate effectively with a wide range of health professionals.
